本保險計畫保險期限為1年，第二年可辦理續保；參加本保險計畫最高投保年齡60周歲，續保可放寬投保年齡至65周歲；
The effective period of the Policy is one year, renewal possible; The insured should be under 60y old. For renewal, the age can be extended to 65y old. |

本会员保險費採用每年交付的方式；
Premium payable annually. |

本保險計畫身故保險金以法定繼承人為受益人；殘疾保險金、醫療保險金及重大疾病保險金的受益人為被保險人本人；
The beneficiary of death benefits for the Policy is legal beneficiary. The beneficiary of Accidental Disability, In-patient, Out-patient and Critical Illness benefits is the insured him/her self. |

如現被保險人現在或過去患有惡性腫瘤、心臟病、中慢性腎功能衰竭、白血病、肝硬化、高血壓II期及以上、糖尿病II期及以上、癲癇病、精神病、中風、再生障礙性貧血、帕金森氏病、血友病、紅斑狼瘡及結締組織疾病、愛滋病等病症須提供個人健康告知聲明書。

保障说明 / The Policy Notice: |

| 1. |
.疾病身故保險金 / Disease Death Benefit：
保險生效日起30天后被保險人發生疾病身故或疾病全殘的，保險公司依照保險合同約定給付保險金。
during the effective period of the Policy, if death or total disability occurs to the insured due to disease 30 days after the effective date of the Policy, the sum Assured will be given to the Insured or the Beneficiary. |

意外身故保險金 / Accidental Death Benefit：
保險有效期間內，被保險人因遭受意外傷害（除特定交通工具意外），自意外事故發生之日起180日內身故，保險公司給付意外傷害身故保險金。
during the effective period of the Policy, if death occurs to the Insured within 180 days since the accidental injury occurs, the Sum Assured as Death benefit will be given to the Beneficiary. |

保險有效期間內，被保險人以乘客身份搭乘飛機、陸上交通工具或輪船時,遭受意外傷害，自意外事故發生之日起180日內身故，保險公司給付特定交通工具意外身故保險金。

特定交通工具意外殘疾保險金 / Accidental Disability in specific traffic accidents：
保險有效期間內，被保險人以乘客身份搭乘飛機、陸上交通工具或輪船時遭受意外傷害，自意外傷害發生之日起180日內導致殘疾，比照殘疾給付等級，保險公司給付特定交通工具意外殘疾保險金。

重大疾病保險金 / Critical Illness Benefit:：
保險生效日起，30天后被保險人經醫院明確診斷初次患有本合同所定義的重大疾病，且生存28天以上者，保險公司將給付重大疾病保險金。

保險有效期間內，被保險人因意外、疾病進行門診或急診治療時，實際發生的合理且必要的門診或急診的醫療費用（包括藥品費、診療費、牙科治療費、檢查費和材料費等）按約定的給付比例進行賠付，每次門診或急診最高給付限額為人民幣800元。其中牙科治療費年累计最高给付1500元。
The insurance company will pay the outpatient medical expenses by signed ‘payable ratio’ to the insured who is treated due to accidental injury or disease occurred during the effective period of the Policy. The outpatient expense paid by the insured must be necessary and reasonable. Reimbursement limit per visit is RMB 800. The reimbursement limit per year for dental treatment is RMB 1500. |

In mainland China, company will pay the in-patient medical expenses by signed ‘payable ratio’ to the insured who is treated due to accidental injury or disease occurred during the effective period of the Policy and 30 days after the effective date of the Policy. The in-patient expense paid by insured must be necessary and reasonable. |

保險生效日起30日后，被保險人在臺灣地區因疾病或意外傷害進行住院治療時，實際發生的合理且必要的住院醫療費用（包括床位費、藥品費、治療費、檢查費、材料費等），在臺灣全民健保賠付後的自付額部分按約定進行賠付。
In Taiwan area, while the rider is in force, company will pay the in-patient medical expenses by signed ‘payable ratio’ to the insured who is treated due to accidental injury or disease occurred during the effective period of the Policy and 30 days after the effective date of the Policy. The in-patient expense paid by insured must be necessary and reasonable and be complementary to the reimbursement of Taiwan people’s social medical insurance plan. |

保險生效日起30日后，被保險人在中國境外及港澳地區因疾病或意外傷害進行住院治療時，實際發生的合理且必要的住院醫療費用（包括床位費、藥品費、治療費、檢查費、材料費等）按中國境內三級甲等醫院醫療費用標準計算並按約定的給付比例進行賠付。
In Hong Kong, Macau and other parts of the world, company will pay the in-patient medical expenses by signed ‘payable ratio’ and by reference to PRC China hospital grade III charge level to the insured who is treated due to accidental injury or disease occurred during the effective period of the Policy and 30 days after the effective date of the Policy. |

關於醫院 / About hospitals：
| (1) |
被保險人在電話報案後，可在國內任何網路醫院或社保定點醫院就診，包括所有網路醫院、特與醫療機構及國營醫院的外賓部、特需部、特診病房（不含重症監護病房）和高幹病房等同類部門或病房。
In-patient and out patient acceptable for all network hospitals and Chinese social security admitted hospitals, including the foreign room area, and other VIP rooms. |

如在中國境外及港澳地區因意外傷害或急性疾病住院治療，可在當地醫院就診。
For emergency treatment in case of accident or of sudden breaking out sickness outside mainland China, the local hospitals are also acceptable. |

如在臺灣地區住院治療，可在當地全民健保局特約醫療機構就診。
In Taiwan, all hospitals admitted by Taiwan people’s social medical insurance plan are acceptable. |

大陸境內指定醫院、中國境外及港澳地區的當地醫院是指在符合當地醫療法規且經核准頒發開業執照並設有病房之全球的公立、私立及財團法人醫療。但不包括專供修養、戒毒、戒酒、護理、養老等非以直接治療疾病為目的之醫療機構。臺灣地區醫院是指當地全民健保特約醫院。
All hospitals defined above should be public hospitals or of similar status, which have local legal license and In-patient rooms. Acceptable hospitals exclude those such as health resort, sanatorium, convalescent hospitals, drug & alcohol abandon center, retirement aged person center, etc whose main purpose are not of traditional medical cure and treatment. |

注（1）：若需接受門診、急診、住院治療，可與醫療服務公司聯繫，由醫療服務公司為您安排醫院、推薦醫師、控制並提高您的治療品質。
In case of need to get access to hospitals, the insured can call up the service hotline. The medical service center can provide help to arrange hospitals, recommend doctors, manage and make easy the whole hospital visit. |

注（2）：参与一年一次免費身體檢查前需電話預約。若未在保險期間內参与并前往指定體檢機構體檢，視同自動放棄。
A telephone appointment is needed to get Medical Check-up. The medical check-up service is only valid during the effective period of the Policy. |

注（3）：乘客是指購票搭乘飛機、輪船、陸上交通工具之乘客，不含該公共交通工具上的工作人員。
Passenger refers to the person who by paying the relevant tariffs takes flight, boats, or other common carriers. Passenger excludes any working staff on the flight, boats or other common carriers. |

注（4）：搭乘是指被保險人開始登上飛機、輪船、陸上交通工具至完全離開該公共交通工具為止。
Taking flight, boats, or other common carriers starts from boarding on the carriers till leaving off the carriers. |

安法会员在中国境内，因身体不适需要有关医疗咨询时，可在任何时间拨打安法24小时医疗服务中心之客服专线，得到医疗咨询服务。此医疗咨询服务仅属建议性质，并不构成临床诊断。
Those ANFA members who need medical consulting when they feel unwell in China may dial the customer service hotline of ANFA 24-Hour Medical Service Center to receive medical consulting service, which, however, is advice in nature and does not constitute a direct clinic diagnosis. |
| 2) |
引导就医 / Referral
安法会员若病情严重需要进一步的治疗，安法24小时医疗服务中心可协助安排至最近且最合适的特约医疗机构接受诊断治疗。
For an ANFA member who needs further treatment in China to address any serious condition, ANFA 24-Hour Medical Service Center will assist in referring the member to the nearest contracted medical institution for clinical diagnosis and treatment. |

医疗监控 / Medical Monitoring
安法会员于中国境内需要住院治疗时，安法24小时医疗服务中心的医疗顾问将密切地与安法会员的主治医师沟通，并随时掌握其治疗情况，及时做出相应处理，必要时将代为通知安法会员之家属。
For an ANFA member who needs hospitalization in China, the medical advisors of ANFA 24-Hour Medical Service Center will keep in close touch with the doctor in charge to keep track of the latest clinical treatment, make timely response and, when necessary, notify the member’s family via telephone. |

医疗转送 / Medical Transfer
安法会员在中国境内，因意外伤害或突发疾病致紧急情况时，《安法24小时医疗服务中心》的医师及会员的主治医师认为该城市之该医疗机构内的技术、设备无法满足会员治疗之条件时，安法专员将为您启动境内转送机制，安排救护车或其它适当交通工具，由一名医师或护士陪同转送会员，从初诊地至邻近最适当的医疗机构，并承担上述转送费用（包含从出事地点到初诊地点合理的运送费用）。
For an ANFA member who has fallen into critical conditions in China due to an accident or disease outbreak, ANFA specialists will activate the domestic transfer mechanism by arranging an ambulance or other vehicle as may be necessary under the circumstances to transfer the member from the place of first visit to the nearest most suitable medical institution under the escort of one physician or nurse, with ANFA bearing all the expenses (including the reasonable costs incurred by the transport from the scene to the place of the first visit), if the physicians of ANFA 24-Hour Medical Service Center and the doctors in charge of the member believe that the technology and equipment of the medical institution of the local city cannot satisfy the needs of treatment. |

健检服务 / Medical Checkup
安法会员可享用一年一次免费基础健康检查，检查前需先提前两天电话预约健康检查时间，以保留属于您的私人时间，节省您健康检查的等待时间，若未能在保险期间内前往指定健康检查的机构接受检查，是同为自动放弃。
ANFA members are entitled to a free basic medical checkup once a year subject to an appointment made 2 days in advance via telephone so as to reserve a period for your own use and shorten the wait. Please note that the failure to receive the medical checkup from the designated institution within the term of insurance will be deemed a voluntary waiver thereof. |

病例建档 / Filing of Case
安法会员医疗服务中心将为每位会员于我司网站上开辟会员病历档案专区，细心地将每一次就诊的记录，如实地载于医疗专区内，会员仅需登陆本公司网站会员专区，输入您的密码，便能够获得您的既往所有医疗就诊记录。
ANFA 24-Hour Member Service Center will open an exclusive area of case files for all its members on its own website to carefully save the record of every visit. A member needs only to log onto the member service area of our website and enter the password to receive all the records of past visits. |

回访服务 / Callback Service
安法会员在就诊结束后一周内，医疗顾问会亲切致电关心其康复情形，并提供正确医疗保健相关知识；如需预约下一次门诊，安法专员会於下次就诊前12小时内，致电提醒会员们准时就诊。
Within one week after the visit of an ANFA member, a medical advisor of ANFA will give him/her a phone call to follow up on his/her health status and provide knowledge about correct healthcare. If appointment needs to be made for a next visit, an ANFA specialist will call the member 12 hours in advance to remind him/her of the need to make the visit on time. |

门急诊费用代垫 /
Advancement of Expenses for Outpatient/Emergency Service:
安法会员于特约医疗机构接受门急诊治疗时，只需要依据该次挂号、检查、治疗、药品、医疗材料的总费用计算，若该次总费用之90%超过800元时，除需自付总费用之10%的费用外，尚需承担超过800元的部分金额。
An ANFA member who receives outpatient/emergency service from a contracted medical institution needs only to pay 10% of the total expenses for registration, examination, treatment, drugs and medical materials. If, however, 90% of the total expenses of the visit exceeds RMB 800 yuan, the member needs to bear the part of expenses in excess of the 800 yuan on top of the 10%. |

住院费用代垫 / Advancement of Hospitalization Expenses:
安法会员于特约医疗机构接受住院治疗时，无需承担与治疗相关的费用（包括：住院保证金、住院病床费、手术费、、），专员会为您提早做住院安排，并通知特约医疗机构启动医疗代垫机制。
An
ANFA member who stays in a contracted medical institution for treatment need not pay treatment-related expenses (including hospitalization deposit, operation fee, etc.). A specialist will make arrangement for your soonest possible hospitalization and notify the contracted medical institution to activate the advancement mechanism.
住院期间，若会员需要安排看护，安法医疗服务中心可为您安排专业医疗看护提供高品质服务（会员需额外负担RMB 500元/天/名）；住院期间使用的任何与治疗无关的设备（如：饮食、电话、网络、停车费、、等等）由此产生的费用需由会员自行承担。
If the member needs a carer during the hospitalization, ANFA can arrange a professional medical carer to provide quality service (incurring an extra of RMB 500 yuan/day/carer to be borne by the member). The member also needs to bear any and all expenses incurred by the use of any equipment irrelevant to treatment during the hospitalization (for example catering costs, telephone and Internet charges, parking fee … etc.). |

会员经安法24小时医疗服务中心安排于境内就诊时，依据金盛人寿保险公司的理赔要求，可进行门急诊或住院医疗费用的代垫付；每例门急诊或住院代垫案件，将从会员保额中额外收取该例门急诊或住院当次所垫付医疗费用总额10%作为服务费（即代垫付服务费计入被保险人个人保额中）；医疗费垫付金额与垫付服务费年累计不得超过保单中所载的个人医疗保险金额。若单笔医疗费用超过年余额或年度医疗费用的合计金额超过年度保额时，安法实业公司将不再提供医疗垫付服务，安法会员的年度保额以保险公司所提供的额度为准。
For any member seeking medical help in China as arranged by ANFA 24-Hour Medical Service Center, advancement of expenses for outpatient/emergency/ hospitalization service may be made according to the requirements of AXA-Minmetals Assurance Co., Ltd. (“AXA”). For each case of outpatient/emergency/hospitalization service, an amount equal to 10% of the total medical expenses advanced for the case will be deducted from the coverage of the member as handling charges (meaning that the handling charges of advancement will be included into the personal coverage). The annual accumulative amount of advanced medical expenses and handling charges shall not exceed the amount of personal medical insurance indicated in the policy. If the medical expenses in both single and accumulative payment exceeds the annual balance, ANFA will not make advancement. The annual balance of an insurant is subject to the amount calculated by AXA. |

For any ANFA member who experiences loss of luggage or delayed delivery of luggage when taking a normal flight for business or tourist travel, ANFA will assist in contacting the related units, e.g. the airline or a Customer officer, to look for the luggage. If found, the luggage will be delivered to the place designated by the member (with the carrier bearing the delivery costs according to international practices). |

For any ANFA member who has any important document (e.g. passport or visa) and/or ID card lost or stolen, ANFA will provide information on reissuance and the local department or institution that undertakes the reissuance (at the expense of the member). |

安法会员因紧急情况不能按原计划的路线继续旅行，将协助会员重新安排航班或旅游路线计划，但不承担任何费用。
For any ANFA member who cannot continue to travel according to the planned route due to an emergency, ANFA will help the member rearrange a flight or tourist route without bearing any expense arising therefrom |

医疗转介服务 / Medical Referral Service
安法会员在境外因突发疾病、意外伤害等原因，需要与当地接受进一步治疗时，安法专员将协助联系《秀传医疗体系或安盛援助AXA》提供医疗服务：
For any ANFA member who needs further local treatment outside China due to disease outbreak or accidental injury, ANFA specialist will assist in contacting the Show Chwan Health Care System or AXA Assistance for provision of medical service
《秀传医疗体系》可安排会员于台湾地区任何一家医疗机构就诊，亦可安排美国哈佛医学中心之12家医疗体系就医；
The Show Chwan Health Care System can arrange for the member to receive medical help in any medical institution located in Taiwan or any one of the 12 medical institutions under Harvard Medical School.
《安盛援助AXA》可安排会员至当地其他医疗机构的医师或专家诊疗，同时协助事先预约（当地所有医疗费用需由会员先自行承担然后再将保险理赔所需的材料交给安法公司，由安法公司代理向保险申请理赔）。
AXA Assistance can make arrangement for the member to receive diagnosis and treatment from any other physician or expert of the local medical institution and help make appointment (with the member bearing all local medical expenses).
安法会员就诊结束时，需向该医疗机构索取诊断证明、收费明细表并带回中国，将上述资料交给安法24小时会员服务中心之客服专员，为其代为向保险公司进行申请理赔（保险公司将依据会员与境外实际发生合理且必要的医疗费用，含床位费、药品费、治疗费、检查费、材料费、、等等，按照中国境内三级甲等医院之医疗费用标准计算，并按约定的给付比例进行赔付）。
At the end of the visit, the ANFA member needs to request diagnostic certificate and fee list from the medical institution, which the member should bring back to China and deliver to the customer service specialist of ANFA 24-Hour Member Service Center. The specialist will represent the member in claiming against the insurance company (which will make compensation according to the agreed proportion of payment for the reasonable and necessary medical expenses incurred by the member outside China, including sickbed charges, drug costs, treatment expenses, examination fees and material costs, which will be calculated according to the medical expense standard for Grade-A hospitals of Class III in China). |

紧急医疗转送 / Emergency Medical Transfer
在境外旅行期间，案发会员遭遇意外伤害或突发疾病时，如《安法24小时医疗服务中心》的医师和当地的主治医师建议应将其转送到境外其他适当的医疗机构住院治疗，安法将安排：
将安法会员转送至最近、最适当的医院；
在安法会员情况允许的情况下，将安法会员以正常的商业航班（经济舱）转送到合适的医疗机构。《安法24小时医疗服务中心》的医师和当地的主治医师将根据安法会员的病情、当地医疗条件、适当的交通工具和安法会员所在的位置决定转送的必要性、时间安排、交通工具和医疗队。符合转送要件者由保险公司承担转送费用。
For any ANFA member who suffers accidental injury or disease outbreak during an overseas travel, ANFA will help transfer the member to the nearest suitable hospital if a physician of ANFA 24-Hour Medical Service Center and the local doctor in charge suggest transferring the member to another overseas medical institution for hospitalized treatment according to the state of illness of the member and local medical conditions |

协助转送回国 / Assistance in Homebound Transfer
由会员的主治医师及《安法24小时医疗服务中心》的医师共同认定，当会员病情或伤势已稳定，可协助安排会员搭乘正常航班（经济舱）或其他交通工具返回中国境内之居住地，安法有权尽可能使用会员初始旅行时所购买的原始回程机票；若该机票需更改日期或升舱等，其所产生的费用由保险公司承担；会员若无原始回程机票，从所在地（国）返回境内的单程机票费用由会员自负；若会员购买的原始回程机票，由于救助过程而过期、失效，将由保险公司承担会员的回程机票费用，但须向会员收回原始机票。
When the state of illness or injury of the member has stabilized as jointly identified by the doctor in charge of the member and a physician of ANFA 24-Hour Medical Service Center, an ANFA specialist will help arrange for the member to take a normal flight (economy class) or other vehicle to return to his/her place of residence in China by doing the utmost to use the original return air ticket that the member bought at the beginning of the travel or bearing the relevant expenses arising from the changed date or upgrading of class of the ticket. If the member has no original return ticket, he/she should bear the fare of the air ticket for the one-way flight from the local country to his/her home country. If the original return ticket bought by the member has become outdated or expired due to the rescue process, the member will bear the fare of the air ticket for the return trip, but the original air ticket should be recovered. |

协助递送基本药物及医疗设备 / Assistance in Delivering Basic Drugs and Medical Devices
安法会员于境外接受紧急治疗期间，经会员的主治医师请求，在法律允许的情况下，协助向会员递送当地无法得到的基本药物和医疗设备（会员需承担药物和设备费用及递送费用）。
For any ANFA member who receives emergency treatment abroad, ANFA will assist in delivering to the member the basic drugs and medical devices unavailable locally at the request of the doctor in charge of member, if so permitted by laws (with the member bearing the costs of the drugs and devices and the freight of delivery). |

紧急口寻传递 / Transmission of Message
安法会员于境外商务、旅游期间发生紧急情况时，可协助尽快通知会员亲属。
For any ANFA member who gets involved in an emergency that occurs during his/her overseas business or tourist travel, ANFA can assist in notifying the member’s family as soon as possible. |

紧急返回居住地国家 / Emergency Return to Home Country
当安法会员境内近亲身故时，如会员在境外旅行途中（不包括移民）需要紧急返回境内，将安排返程并承担其返程机票（经济舱）的费用。
If an ANFA member traveling abroad (excluding immigration) needs to return home in emergency to address the death of a domestic immediate family member, ANFA will arrange the return trip and bear the fare of the return air ticket (economy class). |

亲属探病 / Visit by Family Member
当安法会员在境外商务、旅行途中，因意外伤病或突发疾病，在境外医院接受治疗，且住院时间连续超过10日，可安排一位会员直系亲属从境内到会员入住的医院，并承担往返机票（经济舱）费用及不超过5天的住宿费用（最高额度RMB1200元/天，不包括酒水、饮料和饭店服务费）。
For any ANFA member who stays in an overseas hospital to receive medical treatment for more than 10 days running due to accidental injury or disease outbreak during his/her overseas business or tourist travel, ANFA can arrange for one of his/her immediate family members to visit the member in the hospital from the home country by bearing the fair of a round-trip air ticket (economy class) and no more than 5 days of hotel expenses (≦RMB 1,200 yuan/day, excluding wine, food and hotel handling charges) |

遗体转送回国或当地火葬 / Homebound Return or Local Cremation of Corpse
若安法会员在境外旅行期间，不幸因意外伤害或突发疾病而身故，可以安排运送遗体至北京、上海、广州等国际机场，并负担运送费用及符合航空运输规格的棺木费用，但不承担其他费用。如会员的直系亲属要求且情况允许，可以协助安排在事故地火葬，并负担相应费用及承担将遗骨转回境内居住地的费用，但不承担其他费用。
For any ANFA member who dies from accidental injury or disease outbreak during his/her overseas travel, ANFA can arrange the transport of the corpse back to one of the international airports in Beijing, Shanghai and Guangzhou by bearing the freight and cost of a coffin that meets the specifications for air transport without bearing any other expenses. If so required by the member’s immediate family member and permitted by circumstances, ANFA can also assist in arranging the cremation at the place of accident and bear the relevant expenses, including the expenses incurred by the return of the ashes back to the member’s domestic place of residence. |

协助未满18岁儿童返国 / Assistance in Homebound Return of Child under 18
安法会员未满18岁子女随同会员到境外商务、旅行，因会员遭遇意外伤害或突发疾病，需在境外接受住院治疗，导致孩童无人照顾时，可以安排该孩童（限1名）返回境内居住地，并且承担起返程机票（经济舱）、往返机场交通费用。若该孩童原持有的返回机票无效时，应将该孩童原持有之机票交给《安法24小时医疗服务中心》专员处理。如需必要可雇用一名陪同人员护送孩童返国，并承担陪同人员的交通费用。
If an ANFA member who travels abroad for business or tourist purpose together with a child under 18 needs to be hospitalized abroad for medical treatment due to accidental injury or disease outbreak, thereby leaving the child unattended, ANFA can arrange for the child (1 only) to return to his/her domestic place of residence by bearing the fare of the return-trip air ticket (economy class) and the expenses for the travel from the airport. If the original return-trip ticket held by the child becomes invalid, it should be handed over to the specialist of ANFA 24-Hour Medical Service Center for disposal. If necessary, one person may be hired to escort the child back home at the expense of ANFA. |

休养期饭店住宿 / Hotel Accommodation during Recuperation
安法会员的主治医师和《安法24小时医疗服务中心》的医师认为会员出院后，必须继续接受休养时，可协助会员在出院后立即入住饭店进行休养，并支付连续五天的饭店住房费用（最高额度RMB1200元/天）
For any ANFA member who must continue to recuperate after being discharged from hospital as concluded by the doctor in charge and a physician of ANFA 24-Hour Medical Service Center, ANFA can assist the member in checking into a hotel immediately after being discharged from hospital by paying 5 days of rent (no more than RMB 1,200 yuan/day) |

亲属后事处理 / Funeral Affairs Handled by Family Member
安法会员不幸因为意外伤害或突发疾病而异地身故，可安排一位会员直系亲属或指定代理人前往身故地处理后事，并承担往返正常航班（经济舱）的机票费用。
If any ANFA member dies abroad from accidental injury or disease outbreak, ANFA can arrange for one immediate family member or a designated agent of the member to come and handle the funeral affairs by bearing the fair of round-trip air ticket (economy class) of a normal flight. |

安法会员在中国境内，因为意外伤害或突发疾病时，可协助会员安排特约医疗机构；若该城市内医疗机构的技术、设备无法满足会员治疗之条件，客服专员即启动境内转送机制，安排救护车或其他交通工具，由医师或护士陪同，转送会员从初诊地至邻近适宜的医疗机构，并承担上述转送费用以及从出事地到初诊地的运送费用，医疗转送必须由《安法24小时医疗服务中心》的医师及会员的主治医师共同认定方可启动。
For any ANFA member who suffers accidental injury or disease outbreak in China, ANFA can assist in arranging for the member to be treated by a contracted medical institution. If the technology and equipment of the medical institution in the city cannot satisfy the needs of treating the member, the customer service specialist will activate the domestic transfer mechanism and arrange an ambulance or other vehicle as may be necessary under the circumstance to transfer the member from the place of first visit to the nearest suitable medical institution under the escort of a physician or nurse, with ANFA bearing the expenses incurred by the transfer (including the expenses of transport from the scene to the place of first visit). The medical transfer must be confirmed jointly by a physician of ANFA 24-Hour Medical Service Center and the doctor in charge of the member before being activated. |

医疗转送会居住地 / Emergency Medical Transfer Back to Place of Residence
安法会员在当地住院治疗后，根据会员的主治医师及《安法24小时医疗服务中心》的医师共同认定，其病情以稳定，能转送回原居住地时，协助安排搭乘正常航班（经济舱）或其他交通工具返回居住地（交通工具的选择视会员病情而定）。尽可能使用会员原持有的回程机票，党员持有的返程票无效时，需将原持有的机票交给《安法24小时会员服务中心》之客服专员，另行订票并承担相应费用。会员因医疗需要需使用轮椅、担架或其他辅助设备时，可协助安排并负担相应费用。
For any ANFA member who state of illness has stabilized after treatment in a local hospital and can return to his/her place of residence as confirmed jointly by the member’s doctor in charge and a physician of ANFA 24-Hour Medical Service Center, ANFA will assist in arranging for him/her to return to his/her place of residence by taking a normal flight (economy class) or other vehicle as may be necessary (depending upon the member’s state of illness). ANFA will do its utmost to use the original return air ticket of the member. When the original return air ticket has become invalid, it should be handed over to the customer service specialist of ANFA 24-Hour Member Service Center, who will book another ticket at the expense of ANFA. If the member needs a wheelchair, stretcher or other auxiliary equipment to suit the needs of medical treatment, ANFA can assist in making the necessary arrangement at its own expense. |

安法会员因意外伤病或突发疾病，在非居住地所在的城市住院接受治疗，需要亲属探访时，可安排并承担一位直系亲属往返机票（经济舱）或车票、船票及相应费用。
For any ANFA member who gets hospitalized in a city that is not his/her place of residence due to accidental injury or disease outbreak and needs to be visited by his/her family member, ANFA can arrange for one immediate family member to visit the member by bearing the fair of round-trip air ticket (economic class), or train ticket, or boat ticket and other relevant expenses. |

协助未满18岁儿童返国 / Assistance in Homebound Return of Child under 18
安法会员未满18岁子女随同会员赴居住地以外的城市商务或旅行，因会员遭遇意外伤害或突发疾病，而需住院接受治疗，导致孩童无人照顾时，可以安排该孩童（限1名）返回居住地，并且承担相应交通费用。必要时可安排一名陪同人员护送孩童返国，并承担陪同人员的交通费用。
If an ANFA member who travels to another city this is not his/her place of residence for business or tourist purpose together with a child under 18 needs to be hospitalized for medical treatment due to accidental injury or disease outbreak, thereby leaving the child unattended, ANFA can arrange for the child (1 only) to return to the place of residence by bearing the travel expenses. If necessary, one person may escort the child back home at the expense of ANFA. |

出院后疗养住宿 / Accommodation for Recuperation after Release from Hospital
安法会员的主治医师和《安法24小时医疗服务中心》的医师认为会员出院后，必须在当地进行疗养时，可协助安排饭店，并承担住宿费用（总计最高额度RMB3000元）
For any ANFA member who need to recuperate locally after being discharged from hospital as concluded by the doctor in charge and a physician of ANFA 24-Hour Medical Service Center, ANFA can assist the member in checking into a hotel by paying the rent (no more than RMB 3000 yuan in total). |

亲属后事处理 / Funeral Affairs Handled by Family Member
安法会员不幸因为意外伤害或突发疾病而异地身故，可安排一位会员直系亲属或指定代理人前往身故地处理后事，并承担往返正常航班（经济舱）的机票费用。

紧急返回居住地 / Emergency Return to Place of Residence
当安法会员身处异地时，获悉其直系亲属身故时，需立即折返原居住地时，可安排会员搭乘正常航班（经济舱）或其他交通工具返回居住地，并承担相应费用（该直系亲属身故日必须在责任生效日起180天后）。
For any ANFA member who needs to return to his/her place of residence immediately after learning about the death of an immediate family member, ANFA can arrange for the member to take a normal flight (economy class) or other vehicle as may be necessary under the circumstance at its own expense (the death date of the immediate family member must be 180 days after the day the liability has gone into effect). |

紧急情况随行人员交通费用 / Travel Expenses of Accompanying Person in an Emergency
安法24小时医疗服务中心将安排并支付与安法会员通行的一名直系亲属或指定代理人在安法会员的紧急医疗护送过程中的交通费用，最高限额为RMB10000元。如果已经承担亲属探病的往返机票（经济舱）或车票、船票的费用，将不再承担本项下的费用。
In an emergency, ANFA can bear the travel expense of an immediate family member or a designated agent who accompanies the ANFA member during the emergency medical transfer (no more than RMB 10,000 yuan). But ANFA will not bear the expenses if it has already borne the fair of a round-trip air ticket (economy class), or train ticket or boat ticket of the visiting family member. |

为方便安法会员在事故后向保险公司进行理赔，安法24小时会员服务中心可以协助会员获取保险索赔表格，使得安法会员在事发当地就可以完成理赔手续。
To make it easier for ANFA members to apply to the insurance company for compensation after an accident, ANFA can them in getting the insurance claim form so that they can fulfill the formalities for settlement of claim at the place of the accident. |
